I authorize Brewster Cheese Company and it subsidiaries to use all legal means at its disposal to assess my suitability for employment. I understand and agree that Brewster Cheese Company, any agent acting on their behalf, as well as any other person responding to a reference request pursuant to this application, can and will seek and/or disclose all information about me that said corporation, agent, or persons might have. I understand, also that if employed by Brewster Cheese Company, the first twelve months of employment shall be considered a trial period. As an employee, I agree to abide by all rules and regulations of Brewster Cheese Company. I also understand that nothing in this employment application creates a contract of employment that means that my employment can be terminated, either by me or by Brewster Cheese Company with or without cause, or notice. I understand that no representative of Brewster Cheese Company has authority to enter into any agreement for employment for a specific period of time, or to make any agreement contrary to the foregoing. I understand that Brewster Cheese Company is an At Will employer according to the Brewster Cheese Employee Handbook that means “The employment relationship with the Company is “At Will”; which means that employees are free to quit at any time, for any reason; just as the Company is free to terminate employment at any time, for any reason - with or without notice, with or without cause. Nothing in the scope of employment constitutes a contract or promise of continued employment.”

I understand and agree with the fact that Brewster Cheese Company maintains a drug-free workplace, that maintenance of same is essential to the safety of the workplace and our employees, and that I will be required to undergo a post-offer drug screening. I understand and agree that I may be subject to such testing during the course of my employment, and I specifically agree not to oppose, in any fashion, such pre-hire or post-hire testing. I understand that, subject to applicable law, Brewster Cheese Company shall be the sole judge of the acceptability of any test results. Failure to sign a consent form or cooperate with the testing procedure will result in termination of the hiring process or termination of employment.

I further understand that Brewster Cheese Company complies with OCR 3794 for a “Smoke-Free-Ohio” environment and as such, smoking is prohibited by visitors and employees throughout the interior and exterior premises. Brewster Cheese is a Smoke-Free environment.

I understand that, as a condition of employment, Brewster Cheese Company requires all employees to have their pay directly deposited into their bank account. Because direct deposit is the only option, I agree to produce the required direct deposit information on my first day of employment with Brewster Cheese Company.

I understand and agree that work schedules and requirements vary and can be unpredictable, and that, while Brewster Cheese Company will make reasonable efforts to accommodate work schedules and employee availability, I may be required to work overtime, weekends, different shifts, holidays or other arrangements. I consent to these requirements as necessary and legitimate conditions of employment.

In consideration of the review of my application, I agree that any causes of action or claims that I may have or bring against Brewster Cheese Company shall be commenced within the applicable statute of limitations period, within six (6) months of my knowledge of such claim or cause of action, or within six (6) months after my separation from employment, whichever is earlier. While I understand that the statute of limitations for claims arising out of an employment action may be longer than six (6) months, I agree to be bound by the six-month period of limitations and I WAIVE ANY STATUTE OF LIMITATIONS TO THE CONTRARY. I AGREE THAT ANY CAUSES OF ACTION OR CLAIMS I BRING AGAINST THE COMPANY WILL BE RESOLVED USING ARBITRATION.
